You must live in San Francisco, be age 65 or older, and have annual gross family income at or below 100% of Bay Area Median Income. Check the current limit for your household size. Apply and receive approval to load the free pass onto Senior Clipper; it is not automatically free for every senior.

How to use this benefit
Covers Muni buses, light rail, and cable cars, excluding BART and Caltrain. Applications may take three weeks with an existing Clipper card or about four weeks when a new card is needed. Use the benefit after approval; replacing a lost card may cost extra. The official site confirms the program continues. Check your own pass status before traveling.
